Clearance Packet

CLEARANCE PROCESS

To compete in sports at Grossmont High School the Athletic Packet must be completed in full, accepted and submitted. These documents are required to clear athletes to participate in any sport. You will receive email confirmation which will include three forms

GHS Signature Form
Athletic Clearance Card
Physical Exam Form (Must be completed by physician-physicals are good for one school year)

Complete all forms (signatures and initials where required) and present to the Finance Office to be stamped and cleared prior to the beginning of the sports season. If you intend on playing multiple sports this school year, make a copy of the Athletic Clearance card for each sport. Failure to complete any of these forms will make you ineligible for participation.
